EEOC form 573 serves as an appeal of Final Agency Decision to the EEOC Office of Federal Operations. To send a complaint, you need to complete this form first. This document has to be accompanied with the judge’s decision. A request must be forwarded to the U.S. Equal Employment Opportunity Commission.
Any statement or brief in support of the appeal must be submitted to the OFO within 30 days after filing the request. This document is covered by the Privacy Act of 1974, Public Law 93-597.
